Acarbose has the cleaner mouse signal; humans are unclear on both.
In the ITP cohort, acarbose extends lifespan in mice more reliably than metformin, particularly in males. Human longevity data for both is thin — the MILES pilot of metformin in healthy adults found mixed effects on muscle adaptation to exercise. The current debate centers on whether metformin's blunting of training response makes it a poor longevity drug for active people.
