that is glucose sparing at the level of the muscle which is a very different physiologic state then hyperinsulinemia insulin resistance in which the adipocyte sees lots of insulin but the adipocyte becomes resistant to that insulin signaling because it's already chock-full in that case the adipocyte also releases palmitate but that is creating a negative effect at the level of muscle then the muscle refuses glucose and but it's in a setting of hyperinsulinemia
