I would say about 20% of people are aware of it and that's sort of what's written but again um so there's this whole another theme going on in the sexual medicine literature about whether women have the prostate function in the local genital milu that results in the squirting of fluid um what I can say is it's controversial and probably um more commonly What I Hear in when people are having sort of a more normal orgasmic or arousal and than orgasmic response is that the lubrication from the mucosal surface becomes robust right and that's probably the interaction between vasod Dil ation the nervous system and the local hormones such as VIP and nitric oxide um where the squirting of fluid and where the female prostate actually resides structurally is an area of controversy I think that's the simplest answer and I don't think that's the biggest piece of orgasm we're getting back to that I don't know if you have other thoughts or you know it's it's sort of no I I I I find this to be a totally fascinating topic as any as any male would who's seen all extremes of this uh and and you you you think um you know it's not it's not consistent either right so you wonder is that is that a super orgasm is that uh you know do some women have that every single time I mean it's uh I so I think um a more interesting question I mean I think it's a good one is is because that's not really what people are bothered by it's that they don't feel like they're getting that overall like sensation and peing sens both in their brain and in their genitals it's that sensory experience and the intensity and the muscle contraction that they're probably not experiencing it's not so much they're worried about the lubrication or so to speak the squirting that's not like the big some are and some aren't but the biggest issue is like your patient your your scenario is like that thing doesn't happen this this woman that we're presenting with is this is clearly not the issue that that that's that's that's gerine to her so um what so I I I'm trying to think where to go with this but the first thing I would do is make sure I understand whether this was something she used to have or she let's assume she does let's assume the answer is no this is primary um so so and and the thing about secondary nasm to know is that if a woman has the capacity for orgasm and she loses it one of two things have happened like some kind of significant psychological impact that you need to find out about right could it be trauma relationship struggle something happened and and you want to understand that or there's a physiologic factor like a medication or neurologic condition or something right um and it could be things like my one of my colleagues is really into like the nerve damage from spinning classes right that that not that's doesn't mean don't go out and take get rid of your pelaton but like like in men nerve damage can blunt sensation and may interfere so like I look for those things but we're not talking about that right now right so um the first thing I do is find out like why and look at the context and make sure like somebody's not pressuring her like you know there are people who are like I don't really care but my partner wants me to have an orgasm so I just fakeing and he's bugging me and I'm coming here to see if I can have a real one you know I've heard that right yep okay and um you know sometimes I I I explore it further I never accept the answer at face value I'm I'm big on like finding out more is like have they just given up or they really don't want worry about it right and there are some people in the field who feel like saying well not having an oralism can be normal for some women so you know I avoid that like is it not having an orgasm normal I I sort of say okay well like is this important for you to have this and find out about it and let's learn techniques or strategies for seeing if you can reach this experience right I I don't know what to to say like don't worry it's normal go away like that's you know someone's come to me so we look to see do they are their strategies they could do to have it so there are multiple kinds of orgasms you know the big buckets are cliteral vaginal or both right you know and there's probably like this is where there's maybe some numbers about 30% of women maybe reach or you know if you want to go with rough numbers reach orgasm through P stimulation about 30 through vaginal and 30 have flexibility right um now what techniques for reaching orgasm very widely across women some women can have orgasms just thinking about it right some nipple stimulation you know some women report it with even breastfeeding or like the shower water hitting their nipp some women um need direct clinical stimulation manual oral some women like vibrators um other women through the the thrusting of the Intercourse and like there's again the question where's the G-spot fit in right this spot that's a spongy spot just inside the vaginal Canal on the roof that's a area of sensitivity the bottom line is there's lots of nerve bundles in lots of places and a lot of them can be stimulating enough to trigger this whole mechanism right that's what I tell women and the the big thing for you is to figure out whether you've learned
